Before you make the call:

     ~ You will need the members identification number or social     
         security number and date of birth.
     ~ If the issue is regarding a dependent of the primary, be         
          sure to have the dependents name and date of birth.
     ~ If the issue is related to a claim, be sure to have the claim     
         number, date of claim, and information on the billing party
        (The Doctor, Hospital, Pharmacy, Facility....).

At the end of the call:

     ~ Be sure to document the conversation, what was discussed,
        the name of the person you spoke with, the date and time     
         of the call and ask for a reference number for the call!!!
